Developed by world-class researchers, our Chronic Disease Management Programs can help you take charge of your symptoms and map a sustainable wellness plan. Our programs combine activity and education to target various health concerns and conditions. Choose between in-person or virtual programs to match your lifestyle.

Self-Management Programs
GET BETTER TOGETHER (GBT)
Developed by Stanford University, Get Better Together is a wellness program led by peers living with a chronic health condition or supporting someone with a chronic condition. You’ll develop the tools to understand your condition, manage your symptoms and lead a healthier life.
This free program takes place once a week for six weeks for 2.5 hours. You can take part in one of two formats:
- In person – Join a group of others in person at a host location. Registration is required.
- Virtually – Join others virtually on a secure platform. Registration is required. Programs are not recorded and take place live.

Recommended for Arthritis, Asthma, Diabetes, Cancer, Lupus, Chronic Pain, Stroke, Osteoporosis, Hepatitis, Heart Disease, High Blood Pressure, Anxiety, Depression, Parkinson’s Disease, Fibromyalgia, Chronic Fatigue Syndrome, Kidney Disease, COPD and other health issues
Program Details:
- 6 week program (meet once per week)
- Education on chronic conditions
- Managing pain and emotions
- Breathing and muscular relaxation techniques
- Advice on navigating healthcare and making treatment decisions

Powerful Tools For Caregivers (PTC)
Being a great caregiver means taking care of yourself too. Learn to balance self-care with caring for a loved one through this free, 6-week program. Workshops focus on caring for an adult or a child.
Rate: Free

- Develop self-care tools and caregiver communication strategies
- Learn relaxation and emotional management techniques

Total Brain Health (TBH)
Sharpen your cognitive skills and learn how you can prevent age-related memory loss through Total Brain Health®, an evidence-based cognitive training program.

- Participate in hands-on, engaging activities
- Explore how relationships, outlook and stress impact your brain health

Memory And Aging Program (MAP)
The Memory and Aging Program (MAP) can help you remember what’s most important to you. This 4-week program is designed for people who want to improve their brain health and develop memory skills and strategies.
Rate: Free
Program Details:
- Four, weekly two-hour sessions
- Developed by clinical neuropsychiatrists
- Virtual and in-person options
- Education on lifestyle changes, memory strategies
Outcomes:
- Greater understanding of lifestyle factors impacting memory
- Improved ability to recall everyday things
- Improved memory confidence

Group Sessions
Kidneyfit
A free, three-level program offering exercise and education for people living with all stages of chronic kidney disease (CKD).
Rate: Free
Program Details:
- One session per week over 10 weeks
- Demos from an exercise professional on safe exercise and equipment use
- Assistance in transitioning from supervised to unsupervised exercise
- Health education on living with CKD
Levels and Outcomes:
KidneyFit 1: Get Active – Learn the fundamentals of exercising safely with kidney disease and understand your body, nutrition and medications on a deeper level.
KidneyFit 2: Be Active – A combination of supervised and unsupervised exercise with guidance from fitness experts on cardio, weights, balance and stretching exercises.
KidneyFit 3: Stay Active – In the final level, you’ll gain the confidence to perform solo exercises safely. Upon graduating from the program, you’ll learn how to incorporate physical activity into your life.

Breast Cancer Wellness
Designed for breast cancer survivors, this weekly, supervised exercise program supports the recovery and rehabilitation of arm, chest or breast lymphedema. Offered in partnership with the Breast Health Centre.
Rate: Free (for Breast Health Centre Patients)
Program Details:
- 8 weekly sessions
- Support from certified instructors and manual lymph drainage therapists
- Gentle exercise in our gym and warm water therapy pools
- Connect and rehabilitate with other breast cancer survivors
Outcomes:
- Helps ease side effects of lymphedema
- Improves physical function and boosts mood
- Promotes a safe and gradual return to physical activity
